I accept that if the fault is found on my equipment, caused by my negligence, is due to the loss of Openreach equipment, is located on my premises or land, or if I make and subsequently miss an appointment, then I will be liable for an engineer call out fee of £105.00 inclusive of the first hour and between £55.00 and £85.00 for every hour thereafter dependant on my line type. I accept these charges, have read and fully understand the Fault Diagnosis Guidelines, and now wish to proceed and report a fault.
